There's a loose connection where the cord connects
It is fairly easy to open the gampad by removing the screws. Be careful and make a note of where everything is inside. Do not force it open. The real problem will be fixing the loose connection. The cable has four separate cores (wires) inside and you il need to strip the outer sleeving back to expose the wires. These are all insulated so you will not be able to see easily which wire is broken inside (there may be more than one). If you have a multimeter then you can ceck for continuity for each core from the USB connector to the connector inside the gamepad. If not, then your best bet is to connect it to the PC and fx each core to find the one causing the problem.
Once you locate the faulty core you need to locate where the break in the core is - probably near where it exits the gamepad - then cut through the insulation to expose the copper wire inside. The two broken ends can then be soldered back together and insulated with tape. When putting back together, try to get all the exposed portion of cable inside the controller.

Trying to install Gravis Eliminator Gamepad
I have encountered a similar issue before. What works, is to try the update of your chipset's driver. This could be an error once present in your chipset's initial release, but has since then been fixed by your chipset's manufacturer. Also this could another among the millions of peculiarities in the windows operating system, i say this for it is very possible the the controller will function well if a game is started while the controller is already plugged in. Just see if it works, but an amendment to the chipset's instruction set would be optimal.
