The truck has about 152,000 miles and is in pretty good condition. The A/C stopped about 5 years ago, including the fan. I opened the dash and pulled out the blower control, and that is where I found the wires on the back of the control. The fan has since only worked on full speed or medium, non stop.

I had this same problem and it turned out to be a simple fix,my problem was the hot wire contact on the blower itself.Fix was cleaning the contact w/sandpaper and replacing the part on the hot wire that connects to it.

you need a resister found under your air duct by the passenger floor under dash board auto zone sells these for about 25.00 you may as well buy cabin air filters while your at it . 1 of the screws will be a pain to install back into the duct work I left mine out didn,t make a difference . you might have to order it as they are a comman item oh you will need a vacuum to clean up mess from cabin air filters .this is not a hard job .

These have a real problem with the distributor caps, i would try that
SOURCE: Blower motor or resistor?
I had a truck about the same year with the same problem, it was the wiring harness, but id check the resistor and the fan first before replacing anythings,
