It's not the problem with your phone, contact the customer care of your service provider (network operator), and tell them your problem, they will give you the number instantly. If you enter wrong/random numbers several times, your sim card will be unusable. Good luck

If you need the PUK ( Personal Unblocking Key ) for your phone, you will have to contact your mobile phone service provider.

PUK code is related to the SIM card you are using. So, kindly call the Customer support of your mobile service provider and they will provide you with the PUK code.

Also, entering a wrong PUK code ten times in a row disables the SIM card permanently. So, please be careful while entering the PUK code.

PUK or Personal Unlocking Key is for the sim card and not for the cellphone itself. PUK codes were ask when you give a wrong PIN (Personal Identification Number) for three consecutive attempt. Mostly, PUK codes are found on the SIM casing (It is where you're newly bought SIM was attached). In some service provider has a process it retrieving this codes. However, most of this providers don't have that.
